SMCF Pass-through Donation Application

As a Trusted Community Partner, we already have all your organizational information on file, so this brief application focuses squarely on what SMCF needs to facilitate a passthrough donation.

If, however, your organization lacks internet access, no stress! You can pick up a paper copy at our office, and we’ll upload your typed or handwritten responses for you. Write in the language you are most comfortable with. We understand that every organization is on its own journey, and we’re here to support you as you grow. SMCF funding is available to organizations that are current members of the Nonprofit Network. This step ensures baseline standards are met and allows us to streamline the application process. If you have not yet applied for Network Membership, please complete that first before submitting a Pass-through Donation request.
